After Care Program
2011-2012

We are happy to provide extended care services to our families.  The program can accommodate students in PreK through sixth grade.  Please note the following procedures and regulations.  Extended care is a privilege and your cooperation is required and appreciated in facilitating this service.
Hours of Operation: 2:45 P.M.-5:30 P.M. 

Cost: $40.00 per month, per child or $4.00 per day, per child

Students will report to After Care at 2:45 and check in with the duty teacher.  Any child remaining in car line at 3:10 will be sent to After Care.
Children left in After Care after 5:30 P.M. will be charged $1.00 per minute per child.  Consistently tardy parents may be asked to make other arrangements for their child’s after school care.  There will be no After Care on early dismissal days.

The registration form must be filled out by each family and sent in to your child's teacher. 

For children paying monthly, payments are due on the first of each month and will be delinquent after the fifth day of the month.  Payments should be given to After Care personnel, the child’s teacher, or sent to the office.  Please include the child’s name and grade on all payments.  There will be a $20.00 late fee for accounts that are past due.  Once your account becomes forty-five days delinquent termination of services may occur.
Payment for drop ins will be due the same day of service.

IMPORTANT: All school behavior expectations are enforced at extended care.  Failure to behave appropriately may result in disciplinary action or termination of services by the administration. The After Care Program is intended to serve the needs of the Oak Forest Academy family.  Please help us meet that goal by cooperating with all regulations.
